Pay Rate : $19.50hr. plus an extra $2.00 hr. for hours worked between Friday 11 : 00 pm and Sunday 11 : 00 pm

Schedule : Tuesday 9 : 30am - 6 : 00pm, Wednesday 10 : 30am - 7 : 00pm, Thursday 10 : 30am - 7 : 00pm, Friday 9 : 30am - 6 : 00pm and Saturday 9 : 30am - 6 : 00pm.

GENERAL PURPOSE OF JOB :

Provide guidance, instruction and assistance to ensure maximum growth of each individual toward a semi-independent or independent living situation, whether in Respite, Community Habilitation or a Supportive IRA; assist each individual to reach their highest potential in the areas of their interests, talents and personal goals.

ESSENTIAL DUTIES AND RESPONSIBILITIES :

  • Develop and maintain an effective working relationship with the individuals being served and their family members; provide for the confidentiality of the individuals served and their families; immediately report any accidents, illnesses, behavior or emotional problems and any other unusual circumstances via the program chain of command or as otherwise outlined in program policies and procedures.
  • Provide supports in the following areas : activities of daily living, awareness of and interaction in the community, financial assistance, instruction in emergency procedures and instruction and assistance in food shopping, purchase of clothing and / or other household items as applicable to the type of funded service; supports and instruction provided must be geared to promote personal growth, skill acquisition and community inclusion per the Habilitation Plan or as applicable to the funded service; must be meaningful to the individuals by encouraging and respecting their choices; provide the individual with appropriate social and recreational activities in the community or at home and, when applicable, provide an opportunity for the family to take a break from care responsibilities..
  • Provide a safe and healthy environment while working with the individual in the home, while traveling or when at a community event or location; provide trained responses to behavioral needs per written Monitoring Plan as applicable.
  • As applicable to each individual's needs or stated goals, provide supervision and hands on assistance to teach skills of daily living in the home and in the community, including, but not limited to : meal planning, food shopping and meal preparation, balanced meals and nutritional concepts, preparation of grocery lists and how to shop for food, comparative price shopping, instruction in basic cooking and meal preparation; instruction for independent medication administration and when required count medications and ensure the individual is taking medications as prescribed.
  • As applicable to each individual's needs or stated goals, assist individuals in developing a system to maintain the cleanliness of the home; provide hands on instruction and assistance in personal cleanliness; provide instruction in the use of laundry facilities in and / or out of the home; and if working with an individual that lives in a Supportive IRA, conduct monthly checks for cleanliness and safety as required by regulations.
  • As applicable to each individual's needs or stated goals, assist the individual in the budgeting of personal finances; assist in establishing bank accounts and teach individuals about correct deposit and withdrawal procedures; provide general counseling on money matters; provide guidance and assistance with purchasing items when necessary; and if working with an individual that lives in a Supportive IRA, maintain and complete financial records per regulations, including appropriate use of individual activity funds and maintenance of required receipts.
  • As applicable to each individual's needs or stated goals, provide education and training regarding safety in the community (crossing streets, etc.) and safety within the home (phone numbers for police and fire departments); and when working with an individual that lives in a Supportive IRA, ensure that all safety procedures and regulations are followed, ensure that the procedure for fire drills is understood by each individual and conduct and record fire drills per regulatory requirements.
  • When working with an individual that lives in a Supportive IRA, ensure that all home maintenance functions are completed in a timely and efficient manner : perform routine maintenance and home repair functions and notify the program management staff of all needed home repairs that require landlord or maintenance department intervention.
  • Provide transportation to group and individual events; provide transportation to individuals when necessary or as circumstance warrants; transport individuals to medical, dental and other scheduled appointments that are not in conflict with scheduled programming hours or individual goals; utilize safe driving habits and obey all traffic laws when transporting individuals, conducting Agency business and when operating Agency vehicles or utilizing personal vehicle on Agency business; ensure proper maintenance of vehicle(s) assigned for program use; report all necessary repairs to the program management staff.
  • Act as a member of the program team for the purpose of review of progress and planning for future services; meet with the management staff and the Service Coordinator as needed to discuss each individual's progress, counseling strategies and other pertinent information; communicate effectively with the Supervisor, Service Coordinator, Clinicians and other program staff that also supports the individual to ensure consistency, efficiency and follow-through of services provided; participate in resolving any health, safety or behavioral issues that arise.
  • Using either electronic or paper methods as defined in procedures and as applicable to the funded service(s) that the individual receives, complete and submit all documentation, including but not limited to : verification forms, checklists of service delivery, FYI's, monthly progress notes, Habilitation Plans, medication documentation, expense reports, staff injury reports, telephone and mileage logs, vehicle reports and incident reports by the required due date and for Supervisor review and approval.
  • Have the physical ability to move furniture when required due to an apartment relocation, etc.
  • Provide services to children and adults of all ages.
  • Complete required trainings : discuss the need for additional training in order to meet the needs of the individuals served with the Supervisor; attend and participate in meetings as requested by the Supervisor.
  • REQUIREMENTS :

  • A valid NYS Driver's license or the ability to convert an out of state license within 90 days.
  • Ability to be certified in all required trainings (Trainings are held during daytime hours)
  • Occasionally required to lift up to 50 pounds.
  • High School Diploma or GED (now called TASC) required.
