Job Title: Acute Social Worker
Job Summary: Assist in meeting the psychosocial need of residents/families including but not limited to coping with problems related to illness and disability and utilizing support services available.
**Full Time Day shift position, (which includes weekends) as an acute care Social Worker. Will cover ED and will also assist in the Case Management dept as needed.
Essential Job Duties:
- Duties will include completion of psychosocial assessments with patients/families, knowledge of community resources for referrals, discharge planning for hospice, homeless, or behavioral health patients.
- Coordinates Interdisciplinary Team/Families/significant others: Physicians, Specialists including Dentist, Podiatrist & psychiatrist/psychologist.
- Provides education for new hires and annually including but not limited to abuse & regulatory reporting.
- Serve as a role model for the Vision, Mission and Values of the organization and fulfills other job duties within scope of practice.
- Maintains current competencies as well as updated knowledge in area of practice.

Education/Experience:
- At least 6 months prior experience-BSW highly preferred.
- MSW or Master’s Degree in Human Services related field is preferred.
Licensure/Certifications:
- Current Basic Life Support Card
