Full-time: Monday-Friday
Registered Nurse (Care Manager) – CCHC is seeking a Registered Nurse to join our Care Management team. Applicants should have excellent interpersonal skills, the ability work effectively independently and as a team member, and excellent decision making skills. The care manager will assist with care coordination, communicate with primary care providers, assist with transitions of care, and coach patients to improve their health behaviors. Additionally, the care manager will provide additional support to Providers and assist with wellness exams as needed. Three years of clinical experience is required. Home Health experience preferred.
