Job Summary
The Medical Coordinator will serve as a vital liaison and referral coordinator for Coastal Community Health (CCH) for new lines or service expansion with community partners according to the community needs. The Medical Coordinator will ensure seamless communication, coordination of care, and the implementation of best practices for patient management. This position requires a dedicated professional who can foster collaborative relationships, facilitate patient transitions, and optimize healthcare delivery within the community.
Key Responsibilities
Liaison Role:
· Act as the primary point of contact between referring partnered specialists and Coastal Community Health Center (CCH).
· Facilitate communication and collaboration between healthcare team(s) as instructed by CCH leadership, ensuring alignment of patient care plans and services.
Care Coordination:
· Manage patient intake and system registration, ensuring timely and efficient referral processing and timely patient appointments.
· Ensure registration paperwork is completed including the patient informed consent, patient is provided with CCH information, services, and scheduling follow up appointments.
· Collaborate with interdisciplinary teams of CCH and partnered organization to ensure patient eligibility, provider notes incorporated into CCH electronic medical records with provider signatures, and appropriate claim submission.
Patient Advocacy:
· Advocate for patients’ needs and preferences, addressing any concerns related to care and services with CCH leadership and Partnered entity multidisciplinary teams, and case management services.
· Ensure case management services provide additional education and resources to patients and families regarding available services and support systems.
Data Management:
· Maintain accurate and up-to-date patient records and documentation, ensuring compliance with relevant regulations.
Qualifications:
· Certified Medical Assistant
· Strong understanding of healthcare regulations and standards, including HIPAA.
· Excellent communication, interpersonal, and organizational skills.
· Demonstrated ability to work collaboratively with diverse teams and build relationships with patients, families, and community partners.
· Proficient in electronic health record (EHR) systems and data management tools.
Working Conditions:
· This position may require occasional travel between the skilled nursing center and community health center.
Flexibility in hours may be required to accommodate patient needs and community events
Job Type: Full-time
Pay: $18.00 per hour
Expected hours: 40 per week
