POSITION SUMMARY :
The weekend registered nurse (RN) is responsible for responding to clinical and non-clinical situations for both Home Health and Hospice patients and represents the organization after normal business hours. The RN is responsible for providing skilled nursing assessments, interventions, and supportive care for Home Health or Hospice patients and their families in accordance with the home health or hospice plan of care.
Delivers quality home health and hospice care, assuring compliance with Coastal Home Health and Hospice standards and federal, state, and local regulations. Demonstrates knowledge and support of the philosophy, purpose, goals, and policies of CHHH and consistently complies with the home health and hospice Patient’s Bill of Rights.
ESSENTIAL DUTIES AND RESPONSIBILITIES :
Include, but are not limited to the following :
1. Visits patients that are scheduled for nurse visits on the weekends.
2. Triage calls to home health and hospice agency from patients and caregivers.
3. Assures an accurate evaluation of client needs and develops and implements a comprehensive care plan that addresses those needs.
4. Follows current home health and hospice standards of practice and agency-approved protocols.
5. Makes timely home / facility visits to provide, evaluate and / or change the plan of care based on effective and timely triage assessment of patient and family needs.
Weekend RN
6. May also include recertification and discharge visits that cannot be managed during regular business hours.
7. Communicates with other health care providers and families about changes in the condition and client needs.
8. Demonstrates knowledge of pain management and symptom control and educates patient / family on the physical, emotional, and psychosocial aspects of care, disease process, symptom and pain management, and signs and symptoms of death.
9. Provides for accurate administration and evaluation of medications, management of medication orders, and delivery of medication to patients in a timely manner.
10. Completes home health or hospice admission assessments and develops the patient’s initial plan of care.
11. Communicates response to triage and patient care information in a timely manner to home health or hospice case manager, Clinical Director, and Executive Director of any high-risk or critical issues in a timely manner.
12. Documents patient and family care and necessary changes made to the plan of care to provide current and accurate client records through the use of electronic medical records in accordance with CHHH policies, and federal, state, and local guidelines.
13. Maintains knowledge of, and compliance with, regulatory agencies, accrediting bodies, third-party payers, and HIPAA standards.
14. Remains current in knowledge and practice of home health and hospice standards and regulations.
15. Collaborates with the home health and hospice team including physicians to meet the comprehensive needs of home health and hospice patients and their caregivers / families.
16. Collaborates with vendors to obtain DME, supplies, and medications.
17. If needed, makes arrangements for patient transfers to the hospital by calling report to the hospital.
18. Supervises home health and hospice aides, coordinates other disciplines, and directs patients, caregivers / family members in carrying out necessary changes in the patient’s plan of care.
19. Attends mandatory staff meetings as required.
20. Provides skilled nursing care including but not limited to physical assessments, pain management, administration / supervision of medication, catheterization, dressing changes, irrigations, venipuncture, intravenous infusions, central line care, and diabetic care.
21. Teaches, and demonstrates, basic patient care, use of medications, diagnosis-related information, and treatments to patient / family members / caregivers / representatives.
22. Documents orders received and submits to the physician for signature.
23. Delegates to unlicensed persons per Oregon State Board of Nursing regulations and regularly supervises the tasks delegated.
24. Initiates appropriate preventive and rehabilitative nursing procedures.
25. Completes required documentation (i.e., assessments, care plans, clinical notes, agency tracking forms, etc.) in a timely manner in accordance with agency policies.
26. Participates in the orientation of new staff members.
27. Participates in quarterly record review and quality assurance activities.
28. Other duties as assigned.
Weekend RN
SUPERVISORY RESPONSIBILITIES :
- Exercises supervision of Home Health / Hospice Aides in the performance of duties for patients. Provides written assignments and patient-specific teaching for all care to be given and regularly makes supervisory visits to patient’s home in accordance with state and federal regulations.
- Supervises caregivers performing delegated tasks assigned by the RN. Regularly reevaluates competency in performing the tasks.
