JOB TITLE : Law Enforcement Assisted Diversion (LEAD) Peer Support Specialist / Case Manager
LOCATION : New Hanover County, Wilmington, NC
SUPERVISED BY : Reentry Services Director, Clinical Supervision provided by JS Clinical Director
REQURIED EDUCATIONAL / EXPERIENTIAL QUALIFICATIONS :
High School Diploma or higher, knowledge of recovery, harm reduction and wellness-based concepts.
Personal experience as a consumer of mental health and / or substance use disorder treatment, or some equivalent experience preferred. Ability to complete a peer education training program.
Two (2) years of recovery with mental health or substance use required; PSS Certification / WRAP preferred.
GENERAL STATEMENT OF DUTIES :
The LEAD Peer Support Specialist / Case Manager will provide recovery based supportive services for program participants assisting them in pursuing and achieving health, wellness & recovery goals. Additionally, the PSS / CM will serve as a liaison for community partners and program outreach / systematic mapping.
DISTINGUISHING FEATURES OF CLASSIFICATION :
Assist with navigating treatment systems, connecting with harm reduction services, and recovery planning.
Serve as primary contact for new and existing LEAD clients by coordinating program enrollment and follow-up case management services.
Track program data utilizing web-based electronic medical record platform.
Effectively communicate with community partners to advocate on behalf of clients based on recognized needs.
ESSENTIAL DUTIES AND TASKS : APPROX % OF TIME
Direct Client Services- 65%
Educate clients on the community resources and programming available for justice-involved population.
Assist clients with program enrollment based on identified needs.
Provide individualized coaching to clients in obtaining their health & recovery objectives.
Create and adapt wellness objectives & plans to overcome barriers to good mental and physical health based on needs of, and with input from, clients.
Serve as primary contact for linkage to treatment services, community resources and recovery supports.
Provide transportation and / or coordinate transportation services for clients seeking community-based treatment services.
Indirect Services – 25%
Case management to coordinate services with resource entities and promote consumer engagement in harm reduction and / or treatment-based services.
Maintain program and client records, complete periodic reports in conjunction with program data coordinators, program evaluation staff and program director.
Attending LEAD staffing meetings and effectively communicating with LEAD Coordinating Group to advocate for client services and progress.
With assistance from Coordinating Entity Group PSS will participate in educating the community on LEAD services and / or host community training to target referral sources for programming. Target audience and subject matter for training includes law enforcement officers, attorneys, judges, civilian first responders, paraprofessionals and other professionals on mental health and substance use awareness and identification, de-escalation, diversion, and crisis resolution practices in a culturally appropriate manner.
Staff Development – 5%
Actively participate in assigned activities such as supervision, trainings, grant evaluation and team meetings.
