Would you like to be involved with a team who ensures that every patient and family is honored, supported and comforted as they receive the right care at the right time throughout their journey? In your role as:
Admissions Coordinator with Coastal Hospice, you will:
- Initiate referral process based on agency guidelines. Responds promptly to inquiries about hospice and Compass services.
- Ensure accurate referral and admission information in EMR.
- Ensure continuity and quality of care by expediting the transition from referral to admission.
- Collect relevant patient information from a variety of healthcare sources to add to clinical documentation.
- Obtain physician/nurse practitioner orders and coordinates supportive needs including pharmacy and DME.
- Follow up on referrals previously not taken under care.
- Actively participate in professional development.
- Take verbal orders from providers for hospice admission, medications, etc.
- Receive hand-off reports from discharging facilities, eg: hospitals, nursing homes, rehabilitation facilities
- Enter/call medications into PBM and enters into EMR.
- Assist Volunteer Services and Education Department to administer and check PPD vaccines.
- Assist in processing new admission NOEs and collaborates with the Finance Department to ensure accuracy.
Qualifications
- Excellent customer service skills with both internal and external customers
- Current license to practice in the State of Maryland as a Licensed Practical Nurse required
- Ability to be credentialed as a Coastal Hospice representative in the region’s hospitals
- Solid working knowledge of hospice usually associated with at least 2 years of LPN experience with previous hospice experience, highly desirable
- Working knowledge of basic computer programs, such as Word, Excel, and Outlook mail
- Ability to use electronic medical record system(s), fax machines, scanning, and copiers
