FULL-TIME HOSPICE ADMISSIONS NURSE

In this role, you will:

• Initiate the admissions process for hospice patients by completing the initial comprehensive assessment, coordinating initial services provided by the Interdisciplinary Team, providing medical care, and developing the initial plan of care based on the patient/caregiver/family needs and goals
• Develop, implement and evaluate a plan of patient care utilizing ANA (American Nurses Association) Standards and the Nursing Process
• Utilize equipment and technology skillfully and is technically competent in nursing procedures
• Oversee and/or participates in safe medication handling and practices
• Educate patients and families on significant self-care and end of life issues, all hospice services, patient liability for payment and ensures consent and payer forms are completed
• Acts as a patient advocate. Provide support, information and direct care
• Triage phone calls to identify patient/family needs and notifies Admission Services of referrals
• Oversee and delegate assignments of CMTs, LPNs, Hospice Aides, students and volunteers on shift

• Licensed and currently registered to practice as a Registered Nurse in the State of Maryland, Delaware or other compact state
• BSN from an accredited college preferred
• CHPN certification expected within two (2) years of employment
