PT Night Weekend Hospice Nurse

Are you an LPN or RN looking for a part-time weekend night opportunity?  Coastal Hospice is currently recruiting for PT Extended Services Nurses to work weekends 4pm-midnight

• Provide a consistent high quality hospice experience for every patient and family served
• Triage phone calls to identify patient and family needs
• Monitor and coordinate with extended services staff to complete home and hospital visits as appropriate
• Notify Admissions Services on-call staff of referrals received and obtain pertinent information on patient/family needs and insurance
• Provide direct patient care
• Utilize the nursing process and coordinate with other IDT members to provide for the physical, spiritual, psychosocial, financial and bereavement needs of the patient and family
• Maintain continuity and consistency of care

Qualifications

• Licensed and currently registered to practice as an RN or LPN in the State of Maryland, Delaware or other compact state
• Superior physical assessment skills
• Valid driver’s license with good driving record
• Ability to work independently, efficiently and to make sound nursing judgments utilizing critical thinking skills with a minimum of direct supervision
