Job Position : Physical Therapy Assistant
Location : Bluffton, SC 29910
Pay Rate : $35.00 / hr
Description : Direct Hire : Physical Therapy Assistant (PTA) must have excellent communication and documentation skills and be willing to work with an interdisciplinary team. Help build positive relations within the team and external parties including but not limited to parents, therapists, pediatricians, early Interventionists, teachers, etc. with the ability to establish and maintain cooperative and effective working relationships with others. Be friendly, fun, playful, and social to parents and kids. Responsibilities and tasks :
- Provide 1 : 1 or group PT sessions to individuals birth-21 years. Interventions are to be provided in different natural and non-natural environments i.e. home, community, school, LPS clinics.
- Before the first PT session, check documentation and assess the patient's physical, communication, cognitive skills along with environments of the patient and unique circumstances to develop a treatment plan based on the patient's goals and Plan of Care (POC).
- Consistently providing parents" Summary Sessions form" Fusion Home activities after every PT session (Follow Protocols for Visits)
- Write a daily note after every PT session (must be done within 24hrs).
- Helping children to take part in everyday activities, improving their quality of life by finding solutions to their problems and teaching them to become more independent, based on their treatment plan.
- Giving advice and providing support for family members and Caregivers, regarding participation and carryover in the treatment plan, teaching them skills and techniques to help with the patient's progress (Family Training, HEP)
- Reviewing the patient's progress, adapting the treatment accordingly and writing a Progress Note along with assessment as needed, for those kids who are due for Progress Note every 12 visits. Monitoring a patient's progress to make sure that they're successfully completing the Treatment Plan and if goals need to be modified.
- Perform Re-assessment Packet for those kids who are due for re-assessments, along with an assessment needed for the individual case and look for clinical signs and symptoms to determine effectiveness of treatment plan, creating recommendations to consider.
- Write a Discharge Notes for kids in the caseload as needed.
- When necessary, collaborating with COTA, OTRs, OTDs, ST, PT, supervisors, and additional interdisciplinary staff members (teachers, directors) to create, update or improve students' treatment plan or Sensory Diet (using the necessary equipment's and tools in the classroom for children with needs and disabilities and teaching them how to use it).
- Prepare PT work's areas and maintain neat, clean, safe, and organized. (Follow Protocols for Visits).
- Assure safety for self and others in all spaces. Equipment safe and working properly. All precautions and contraindications to be adhered to.
- Create an appointment every 7 weeks for Supervision Meeting, confirming the Supervision Meeting and make sure this happens consistently, to assure strategies and activities are being utilized appropriately to meet the goals of children on caseload (have all Supervision Forms filled out for the meeting).
- Attend monthly meetings with the team.
- Follow & Check your Fusion Schedule several times throughout the day.
- Confirm sessions the day before with patients.
- Reschedule makeup sessions, for openings after confirming sessions or lastu0002minute cancellations. Supporting functions of the job include, but are not limited to :

- Timely documentation and effective use of time.
- Keep productive in the number of PT sessions you must have weekly, taking into consideration the time you must invest in driving and documentation.
- Consistent flexibility and adaptability to reschedule missed sessions and promote productivity.
- Maintain a professional demeanor and work ethic. Continual company representation with adherence to dress code.
- Adhere to dress code and other requirements of the facility (Follow Protocols for Visits / Dress Code Form Policy)
- Must have the ability and proven ability to report to work on a regular and punctual basis.
- Perform all other related work delegated or required to accomplish the objectives of programs.
- Knowledge and implementation of relevant materials and accommodations.
