Coastal Opportunities, a non-profit agency providing services to adults with intellectual disabilities, is accepting applications for a Licensed Social Work Consultant. This is an excellent professional opportunity to work with a team that positively impacts peoples’ lives.
This position provides social service consultation to our 8 bed ICF-IID group home in Rockland. This Requires Up to 32 hours of services annually, 8 hours per quarter.
Duties include:
· Participating in admission and discharge.
· Participating in the Person Centered Planning Process on an annual basis for each resident.
· Supervise the Social Services Aide and Social Work practices at the home.
· Consult with team members on emerging issues related to programming and individual resident’s needs.
· Supply written report to the Administrator to account for consulting time and to meet state and federal requirements.
· Provide staff training and support as needed, to be determined by consultation between the Administrator and the Social Work Consultant.
Pay Rate of $60 per hour
