Job Overview
We are seeking a dedicated and compassionate Case Manager to join our team. Duties
- Conduct comprehensive assessments to determine client needs and develop personalized service plans and to determine appropriateness for admissions.
- Provide crisis intervention and support to clients facing immediate challenges.
- Facilitate access to community resources, including Medicare services, and medicaid, supplemental security income
- Supervise casework activities and ensure compliance with organizational policies and procedures.
- Collaborate with multidisciplinary teams to coordinate care and support for clients.
- Maintain accurate documentation of client interactions, progress notes, and service outcomes.
- Advocate for clients' rights and needs within the community and service systems.
Experience
- A background in social work or related fields is preferred, 1 year of experience rquired.
- Familiarity with crisis intervention techniques
- Previous mentoring or supervising experience is beneficial to effectively guide clients through their journeys.
- Strong communication skills are essential for building rapport with clients and collaborating with other professionals.
