SUMMARY: Provide a mixture of individual, group, and activity therapy components oriented toward development of individual independence in community life and in the management of illness and recovery for persons with functional limitations due to psychiatric and substance use disorders. Treatment activities focus on the areas of daily living, community integration, and interpersonal functioning.
ESSENTIAL RESPONSIBILITIES AND TASKS:
- Provide clinical services within scope of practice as indicated in the individual’s service and support (treatment) plan and directed by the multi-disciplinary team. Provide direct services as percentage of total allocated work per week at levels consistent with agency policy.
- Provide structured clinical training and development activities, including medication self-administration, meal preparation, money management, basic hygiene guidance and other similar activities.
- Provide medication management services including education and training to assist the individual, family, or other caretaker in the proper management of prescribed medication regimens, drug interactions, and side effects.
- Make accurate and complete notations in the electronic medical record documenting significant information concerning the individual and summarizing progress made relevant to objectives and activities outlined in treatment plans.
- Conduct risk assessment and follow written crisis plans as needed. Assist patient resolution of the crisis or address other needs of the individual to assist in accessing crisis services, seeking supervision proactively.
- Follow Federal, State, local, and agency policies and regulations to prevent abuse, mistreatment, or exploitation of the residents, and to safeguard residents.
- Actively participate in and positively contribute to staff and team meetings as assigned by supervisor.
- Travel and provide transportation for patient appointments.
- Provide supportive case management services as needed and required by the treatment plan. Case management services include coordination of care activities, assistance with housing and employment or vocational tasks, advocacy, resource acquisition, and other similar activities designated or assigned.
- Provide Residential Counselor services in conjunction with direct clinical services and to support the residential facility. (See Residential Counselor job description CL20).
- Actively engage in the creation and implementation of a professional development plan to include a minimum of 16 hours of pre-service orientation and 8 hours of in-service training annually during employment.
- Maintain professional, ethical and behavioral standards of conduct.
- Ensure a safe and sanitary environment for patients by assuring cleaning standards are met. Report any facility and community needs to an administrator or program supervisor in writing by end of shift.
- Follow Federal, State and agency confidentiality procedures.
- Report and document any and all incidents following agency procedures.

- Must have current driver’s license and be able to drive agency vehicle with patients while on daily appointments, able to drive multi-passenger van, and pass DMV background check.
SECONDARY RESPONSIBILITIES AND TASKS:
- Provide coverage for other positions as needed on short-term basis.
SUPERVISORY RESPONSIBILITIES:
This position has no supervisory responsibilities.
REQUIRED EDUCATION AND EXPERIENCE:
- Must have Bachelor’s Degree in Social Work, Psychology, or Sociology OR a combination of at least 3 year’s related work, education, or training in mental health and/or addiction.
COMPETENCIES AND SKILLS:
- Use a strength-based and motivational approach in engaging and supporting individuals in individual service and support plans (clinical treatment plans) objectives and championing recovery efforts.
- Able to articulate basic medical terminology, understand and implement nursing delegation duties, and demonstrate basic understanding of substance dependence and co-occurring disorders on recovery and functioning.
- Able to participate and work collaboratively in team environment.
- Flexible and responsive to changing residential needs.
- Ability to utilize both oral and written communications skills to effectively communicate with residents, co-workers, community resources, supervisors, and various other individuals encountered in the course of performing work while maintaining patient confidentiality and establishing professional rapport.)
- Ability to operate a computer, utilize basic MS Office programs, and effectively navigate and maintain residential records in an electronic medical records system.
- Ability to maintain constructive interactions and demeanor, even in challenging situations.
- Must possess independent problem solving skills.
- Ability to recognize, take preventative measures, de-escalate, and appropriately respond to individuals in crisis and crisis situations in the residential and treatment environments.
REQUIRED CERTIFICATIONS AND LICENSES:
- Valid Oregon or Washington Driver's License.
- Must be credentialed by agency as a Qualified Mental Health Associate (QMHA).
- BLS certification or ability to acquire within 90 days of hire.
- Crisis prevention certification (such as CPI) within 90 days of hire.
