Location: RI ACI in Cranston RI

MAJOR DUTIES AND RESPONSIBILITIES:

· Meet with patients to assess their needs while discharging from Court and/or the Department of Corrections

· Act as liaison to community MAT providers, and attorneys, case managers to ensure continuity of care

· Maintain patient treatment record in SMART and or the RIDOC EMR, including data entry and patient notes

· Daily tracking and organization of patient release dates

· Assemble resources related to reintegration to the community for patients support

· Provide comprehensive case management and direct assistance to obtain the necessities of daily life (i.e. food stamps, insurance, housing, transportation)

· Collaborate with RIDOC discharge planners to facilitate patient reintegration to the community

· Provide aftercare follow up and outreach upon re-entry for 30 days.

· Demonstrate understanding of and adherence to appropriate ethical behavior as stipulated in the CODAC Policy and Procedures Manual, and in accordance with RI Department of Corrections Policy

· Demonstrate understanding of and adherence to confidentiality regulations as stipulated in the CODAC Policy and Procedures Manual, Federal Regulation 42CFR part 2, and other pertinent Federal and State laws and regulations, including RI Department of Corrections Policy

· Obtain and sustain Motivational Interviewing proficiency

· Participate in Performance Improvement activities

· Perform other duties as required

QUALIFICATIONS:

Bachelor’s Degree preferred or a minimum of 1 year experience in the field of addictions or criminal justice preferred. Completion of Peer Recovery Specialist Training. Exceptional organizational skills.
