Description
Under the supervision of the Behavioral Health (BH) Clinical Director for School-Based and Community Services, the Behavioral Health Clinician (BHC) functions as a core member of Codman’s School Based Health Center located at Randolph High School.
The BHC provides consultation/assessment and bio-psychosocial evaluations on an urgent basis, support and linkage services, advocacy, education, brief interventions, counseling, and information to ensure adequacy of BH services.
The BHC will have a strong background in pediatric and adolescent mental health and serve as a consultant for the multi-disciplinary care team, and other personnel around this topic. The incumbent will hold family sessions as appropriate, in their work with minors.
The BHC will provide age-appropriate, evidenced-based interventions for Randolph residents across the lifespan, and to patients of the Codman Square Health Center.
Qualifications
- Master’s Degree in Social Work or related Human Services field (Required)
- Massachusetts License LCSW,LICSW, LMHC (Required)
- Proficiency in speaking Haitian Creole or Spanish (Preferred)
- Provide culturally sensitive assessment, consultation, and brief interventions for psychological/psychiatric problems and/or disorders and diagnostic clarification utilizing DSM5-TR.
- Formulate client-centered treatment plans on assigned cases and engage parent/guardian in the process for minors. Will provide short-term treatment.
- Administer behavioral health screeners to patients, according to Department of Public Health regulations.
- Evaluate crisis situations and apply appropriate interventions and treatment planning (Suicidal/Homicidal Risk Assessment).
- Assist the school-based care team in developing care-management processes such as guidelines, disease management techniques, case-management, and patient education to improve self-management of chronic diseases.
- Facilitate consultation between the multi-disciplinary care team, School Nurse, School Personnel and Parents/Guardians (as appropriate).
- Facilitate initial connection between school and/or community-based social service agencies and state agencies for management of students.
- Participation in Clinical Treatment Team to assist with treatment planning to provide comprehensive care for students and families.
- Cultivate professional relationships with onsite care team and school personnel to identify BH concerns and problems with students.
- Assistance with outcome measures and data collection (when requested).
- Documentation of services within the EHR, ,according to CSHC documentation guidelines.
- Provide an array of clinical services for pediatric and adolescent behavioral health problems and areas of behavioral medicine (treatment planning/provision) to students with coping mechanisms and meet goals.
- With implementation of medical services, the BHC will provide diagnostic assessments/evaluations of adults, and will formulate client-centered treatment plans for those assigned cases.
- Work evening hours, depending on operational needs.
- Attend trainings, events, and meetings.
