JOB TITLE: Clinical Supervisor/RN

DEPARTMENT: HH and Hospice (HHA Admin)

REPORTS TO: Home Health and Hospice Director

JOB SUMMARY

The Clinical Supervisor/RN will ensure quality and safe delivery of home care services and assumes responsibility for the day to day clinical operation of the Home Health & Hospice program. The Clinical Supervisor will support and facilitate the administrative activities of the Home Health & Hospice Department.

The staff member must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. The individual must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his or her age-specific needs, and to provide the care needed as described in the unit's/area's/department's policies and procedures.

JOB REQUIREMENTS

Current Wyoming licensure as a Registered Professional Nurse. Prefer Bachelor of Science in Nursing.

EXPERIENCE

Management experience preferred. One-year Home Health/Hospice experience required.

SKILLS

Knowledge of home health care and/or public health nursing. Able to perform effectively in highly stressful situations. Must be able to write policies and procedures, and interact effectively with a wide variety of people. Excellent communication skills. Able to assume a leadership role and serve as an example to other employees.

ESSENTIAL FUNCTIONS

  • Assume administrative duties in the absence of the Director or QI Coordinator.
  • Coordinate the patient admission process for home health/hospice with the Medical Director and the primary care physician. Oversee the home health and hospice nursing admission process and the implementation of patients Plan of Care including scheduling oversight.
  • Provide hospice benefit explanations and inpatient consultations to potential patients.
  • Supervise department Nurses and Aides and provide for support and resources for Home Health and Hospice nursing staff.
  • Assess department staffing patterns to ensure staff/patient needs are fulfilled. Coordinate and assign new referrals.
  • Develop, implement and evaluate client plans of care per department policy and procedure incorporating Oasis.
  • Attend hospital interdepartmental meeting.
  • Conduct Home Health client care conference meetings every week. Conducts hospice interdisciplinary team meetings every 2 weeks. Complete IDT Form.
  • Manage team schedule with a focus on assigning visits with RN/Case Manager.
  • Collect deposits for room and board for Hospice House.
  • Monitoring compliance with physician orders, face to face documentation and certificate of terminal illness.
  • Communicate regularly with appropriate personnel regarding client care and nursing practices.
  • Assist Director with updating clinical policies and procedures.
  • Assess clients for appropriateness of Home Health and Hospice Services.
  • Assess Home Health/Hospice department staff members’ job performance/competency on a regular basis. Develop staff competencies and assist with systematic assessments.
  • Consult with staff regarding Home Health/Hospice nursing concerns and practices.
  • Comply with all federal, state and local regulations. Keep staff apprised of all new developments. Comply with Medicare Conditions of Participation.
  • Perform patient visits PRN. Nursing assessments and patient evaluations as needed.
  • Complete 24 hours of in-service training per year.
  • Assist in providing on-going education and resources to all staff. Assist with annual education calendar.
  • Verify private insurance and obtain authorizations as needed. Case Manage home health/hospice patients as needed in collaboration with insurance companies and staff.
  • Assume primary responsibility as Home Care Super User System. Tasks include:
    • Along with the IS department, provide Home Care support and user training
    • Ability to initiate and facilitate problem-solving processes when issues identified
    • Effectively study and determine alternate system approaches for meeting user requirements
    • Coordinate with IS department to initiate changes within the scope and authority
    • Serve as the main application systems contact person representing the homecare hospice department for ongoing maintenance thereafter
  • Rotating weekend/holiday call to assist staff.

SECONDARY FUNCTIONS

  • Maintain adequate and appropriate supplies and equipment at all times.
  • Promote a positive work environment.
  • Participate jointly with the Director to interview, hire and orient new nursing staff.
  • Provide orientation and in-service training programs.
  • Participate jointly with the Director in disciplinary actions, if necessary. Submit all documentation to the Director.
  • Attend in services that pertain to department.
  • Develop next monthly call schedule by the 20th of the month with Director.
  • Assure that all staff members comply with infection control procedures.
  • Liaison to NWC Nursing Students.
  • Assist in promoting community education.
  • Participate in community education programs to promote professional growth, understanding of hospice care, etc.
  • Participate in the development of programs for the community, including the memorial service, education and fund raising to assist in the community wide awareness of issues related to death and dying.
  • Other duties as assigned.
