We are seeking a Hospice RN Case Manager
Flexible days / hours - Part Time
Pay Range : $42.00-$45.00 DOQ

Responsibilities :
- Demonstrates excellent verbal and written communication skills with patients / families and fellow peers
- Demonstrates leadership skills in development and updating the patient's plan of care.
- Demonstrates empathy in the delivery of patient-centered care
- Completes, maintains, and submits accurate and relevant timely clinical notes regarding patient's condition and care given.
- Provides professional nursing care buy utilizing all elements of the nursing process.
- Assess and evaluate patient status.
- Completes nursing assessments utilizing critical analytical skills with strong support from the IDG team
- Individualized orientation based on previous experience
- Initiate and revise patient plan of care.
- Completes, maintains, and submits accurate and relevant clinical notes regarding patient's condition and care given.
- Participates in on-call duties as defined by the on-call policy.
