Coleman Health Services is seeking an individual to provide case management services to individuals with severe and persistent mental health issues, working within Portage County, Ohio. The Housing Specialist will be responsible to complete housing assessments and manage several grants related to housing and housing resources. The Housing Specialist will make referrals for housing, employment, and link the individual with ongoing mental health services.

Qualified candidates for the Housing Specialist:
- Three years’ experience in mental health or related field orBA/BS in mental health or related field
- Have/maintain excellent driving records
- Have or willing to obtain NPI and Ohio Medicaid numbers
- Experience in case management preferred
