What are the responsibilities and job description for the Case Manager, Youth Mobile Response Team position at Coleman Professional Services?
We are seeking a case manager to assist with rapid engagement and crisis stabilization of youth and families seen by the Mobile Response Team in Belmont, Harrison and Monroe Counties, Ohio. Emphasis is placed on keeping them safe in their nature environment, increase coping skills, accessing/navigating community resources, and linkage to outpatient services.
Duties include:
-Providing short-term, community-based case management to assigned families
-Safety planning and increasing healthy coping skills
-Developing treatment plans
-Assisting with accessing supports, resources, and outpatient services that will help stabilize the crisis and keep them safe in the community
Hours: 11:30 am - 8:00 pm
MRSS Case Managers at Coleman work as a part of a cohesive, multi-disciplinary care team to provide the best outcomes for the clients and families we serve. The MRSS Team provides outreach to youth & families under 21yrs old to help them resolve a person-defined crisis and link with ongoing support services as needed.

Qualified candidates will have their Bachelor's degree in a related field or three years related experience (includes internships and volunteer experience); have and maintain an excellent driving record; and be familiar with child development, family systems, trauma informed care, community resources, motivational interviewing, crisis intervention, and case management approaches. Prefer someone with prior case management experience.
