Join us as the Manager of OhioRISE Case Management/Care Coordination where you will lead a Care Coordination team that drives positive change in the lives of youth and families within our community. This is a full-time position based out of Mansfield, Ohio that will be serving Richland County. Your Impact as a Manager of OhioRISE Case Management/Care Coordination:
- Provide strategic oversight for structured service planning and care coordination, centered on high-fidelity wraparound principles.
- Develop and supervise a team of eight Care Coordinators.
- Actively engage and develop relationships with child-serving systems and behavioral health providers.
- Initiate quality improvement initiatives that align with OhioRISE program requirements.
- Support the development of the local system of care serving youth and families.
Qualifications of a Successful Manager of OhioRISE Case Management/Care Coordination:
- Licensed Social Worker (LSW), Licensed Professional Counselor (LPC), or higher.
- Demonstrated supervisory or managerial experience in community-based services.
- Bring a minimum of 3 years of experience in children's mental health or related public sectors, providing community-based services to children, youth, families, or caregivers.
- Display experience in managing complex cases involving multi-system youth.
- Monitor timely and accurate documentation and coach staff to performance.
- Maintain an outstanding driving record.
- Ability to obtain NPI and Ohio Medicaid numbers (assistance available).
