Colfax Nursing and Rehab is a 140 bed long term care facility with a stable staff and management team.
RN or LPN MDS coordinator needed for fulltime position in a 140 bed skilled nursing facility. MDS coordinator will be responsible for completing MDS assessments and care plans for skilled and long term care residents. Knowledge of the RAI manual is a must. Previous experience with MDS assessments and care plans is required. Benefits include health, vision, and dental insurance; paid days off; and supplemental insurances. Ideal candidate must be flexible and a self starter who is able to work well under pressure. Day shift position Monday - Friday with rotating call schedule. Only serious applicants with experience please.
