Position Purpose

The proposed Homeless Youth Rapid Rehousing Program (HYRRHP) will serve 20 youth, ages 18 to 24 in the District of Columbia. Specific services to be provides include housing assistance, housing search/navigation, case management, family reunification, job readiness/workforce development, financial literacy /credit counseling, legal services to aid with landlord: tenant concerns, and follow-up services. The program is designed to offer each participant a full package of services that directly address the deficits the participants are experiencing that preclude them from obtaining and retaining stable housing. The ultimate goal of the HYRRHP is to establish the participating young people in stable housing with the skills, resources, and support in place to ensure housing retention.

Principle Accountabilities

The program goals are as follows:

  1. Provide housing services to homeless and precariously housed District of Columbia youth,
  2. Increase the capacity to maintain housing among the participating youth,
  3. Facilitate family reunification with all participants,
  4. Increase social support and community connectedness among the participating youth, and
  5. Establish a sense of stability among the participants.

The program seeks to achieve six major objectives:

  1. Place at least 17 of 20 youth (85%) in RRH units,
  2. Exit at least 16 of 20 youth (80%) to permanent housing,
  3. At least 80% of the participants remain housed permanently (at least 12 months after being placed).
  4. Of the young persons who complete the job readiness training, place at least 85% in part- time or full-time employment,
  5. Increased knowledge of financial literacy reported by 90% of the participants,
  6. Increased social support reported by 80% of the participants

Clinical Services- Conduct on-going assessment of the family’s strengths and progress towards independence or permanency goals. Effectively plans and implements interventions in the progress of case planning. Ensure that licensed supervision is provided for all clinical services. Make appropriate referrals to alcohol and substance abuse treatment, mental health services, and supports clients to access public benefit programs and based on assessment need and will of the client to engage in services.

Collaboration - Maintain relationships with other community agencies, educational institutions, businesses and service providers to advocate for clients. Participate in internal and external meetings in spirit of mutual support of homeless individuals advocate for needed resources and services and to ensure clients are provided with all possible resources available.

Supervision – Ensure that all progress notes of engagement activities, client contacts, and clinical notation are recorded in the client electronic record within 48 hours of service delivery in either QuickBase or HMIS, depending on the nature of the information. Review files for each participant ensuring the requirements documents have been completed and documented. Review current skills of Case Managers, provide appropriate work assignments and case supervision. Identify strengths and deficiencies and develop a professional development plans with assigned staff.

Program - Adhere to all program requirements outlined in the grant. Submit a case management monthly report to the District by the 10th day of each month (reflecting activities for the previous month). The report should include all information outlined in the grant. Identify gaps in services to the participants and suggest methods or plans to address gaps in services. Ensure the appropriate annual trainings are completed and documented for program audit.

New Staff – Provide leadership, and actively engage new staff in developing knowledge of CSC, the strengths-based approach, and the framework in which community based social work practices are implemented. Acts as a resource for new staff by providing guidance and support when necessary.

Education

Licensed Professional Counselor (LPC) with a minimum of a Master’s Degree in Human Services or Social Work, or Bachelor of Science in Nursing.

Experience

3 years professional experience in providing counseling and case management to individuals experiencing homelessness or other related populations. Excellent problem solving, analytical, organizational, and oral and written communication skills. Ability to prioritize work.
