Responsibilities
">- Conducting direct street outreach activities to identify and engage unsheltered homeless individuals.
- Building relationships with individuals experiencing homelessness and providing advocacy and support in connecting them to essential services and housing resources.
- Providing peer support and knowledge of transitional and permanent supportive housing to help individuals stabilize their situations.
- Establishing positive relationships with individuals who may have high vulnerability due to mental illness, substance use disorders, or involvement with the criminal justice system.
- Maintaining accurate records and data on outreach efforts and client progress.
Requirements
">- A High School Diploma or GED is required.
- Experience providing peer support is essential.
- Knowledge of HMIS software is a plus.
- Ability to work with individuals with mental illness in a caring and professional manner.
- Proficient in MS Office products and general computer literacy are required.
- Bilingual skills are a plus.
- Valid NJ Driver's License with an acceptable driving record is required.
