College Hospital is looking for 3 per diem Social Worker's to join our team! New Grads Welcome!
The Social Worker will work with patients to promote positive mental health and emotional well-being. The ideal candidate will have a passion for helping others and a drive to make a difference in someone’s life.
Responsibilities:
- Completes Psychosocial assessments on patients on assigned unit according to the guidelines set forth in the Procedures for Psychosocial Assessment.
- Participates in Treatment planning for assigned patients.
- Regularly consults and complies with all mandated reporting laws concerning child abuse/neglect, elder/dependent adult abuse/neglect, domestic violence, and Tarasoff notifications in a timely manner.
- Regularly acts as a liaison between the hospital and other agencies and the court system when a patient on their caseload is so involved.
- Arranges with outside agencies to coordinate a discharge and aftercare plan to meet assessed needs.
- Anticipates patient needs and provides advisement to multidisciplinary team regarding meeting those needs.
- Demonstrates knowledge of assigned cases; to include, but not limited to the current clinical status, discharge plans, and legal status.
- Properly demonstrates behavior modification techniques.
- Provides individual, group and family therapy for patients when needed.
- Submits completed temporary conservatorship applications in a timely manner.
- Effectively performs additional duties and responsibilities as position requires.
Education & Experience:
- Master's Degree required.
- May be a ASW, APCC, LPCC, LCSW, MFT or registered AMFT.
- Social Work training and experience preferred.
- Fluent in Spanish preferred.
