We are seeking a skilled Licensed Practical Nurse to join our dynamic team at College Park Family Care. As a Phone Triage-LPN, you will have the opportunity to work closely with patients and colleagues to deliver high-quality, patient-centered care.
Responsibilities:
Screen patients for eligibility and determine the need for further evaluation or treatment. Prepare patients for exams, collect necessary information, and obtain vitals. Provide accurate and timely information to patients regarding test results, follow-up appointments, and medication instructions. Maintain accurate and up-to-date patient records, including chart entries and posting lab/test and examination results. Arrange referrals to specialists and obtain pre-authorizations when directed by the provider.
Requirements:
Graduation from an accredited school of practical nursing. Minimum of 2 years experience in a hospital or clinic setting is preferred. Must be licensed as a Licensed Practical Nurse in accordance with state regulations.
