The position reports directly to the Intensive Services Access and Diversion Supervisor within the BH division and will work closely and provide support to all Intensive Services programs.
This position connects individuals from the community to LifePath Systems services. This position uses initiative and a passionate commitment to connect individuals to care as well as diverting from emergency rooms, hospitals, and jails. Access and Diversion responsibilities include ensuring individuals receive timely access to appropriate services, working with the entire LifePath Systems team and other community organizations. Must be effective at troubleshooting and organizing support, crisis de-escalation, and linking to community resources. Provides services and care that may include transportation, assisting the individuals served with utilizing daily living skills, preparing meals, achieving personal outcomes, and monitoring to ensure safety. Other duties may include medication monitoring, vital signs, specimen collections, and re-direction to appropriate behaviors.
This position exemplifies service excellence by providing an overview of LifePath Systems services and then connects to outpatient behavioral health services to include crisis services as needed. Engagement will be provided with a trauma-informed and person-centered approach. This position requires accuracy, attention to details, initiative and a pro-active approach to duties and responsibilities while working with the individuals in services, clinical team, and co-workers.
This position practices stewardship of resources by using time effectively with those served and seeks opportunities to expand access efforts. In addition, they ensure all required trainings are completed prior to the due date and center-issued equipment is kept in good repair.
Integrity is necessary to ensure adherence to the Health and Human Services Commission, Texas Administrative Code, contracts, applicable grants, UM guidelines, and Lifepath Systems policies and procedures. This position will work closely with Adult Fidelity managers and the Program Administrator to ensure that all gaps are reconciled.
This position builds positive relationships with local police agencies, social service organizations, and mental health professionals in an effort to increase referrals from community stakeholders and partners. This position demonstrates professional conduct with Peers, Leadership and Support teams at all times. In addition, close working relationships and professional conduct while coordinating care with outside entities is required.
Working independently and collaboratively to maintain appropriate boundaries and foster better outcomes for individuals served and facilitating improvement in measurable ways, is central to LifePath Systems mission to serve. This position must have the ability to assess and de-escalate behavioral health crisis situations in a calm and respectful manner.
Specific Targets: Engagement with individuals served and exceptional customer service will be provided and documented in the electronic health record. 90% of documentation is signed within 2 business days of service. Maintains the ability to assess and determine which direct care practices best meet ongoing needs and help track the progress toward recovery goals for persons served through thorough communication and documentation. This position assists in linking to medical, social, vocational, financial, educational, and other individualized services with a
focus on access to care and diversion from incarceration. Must understand criteria for LifePath Systems services and be able to explain to individuals served as well as community partners.
If Bilingual and receiving stipend, employee will be available to translate for other staff as needed and will utilize approved language in the provision of services as needed.
Qualifications
- High school diploma or GED required.
- Experience working with individuals diagnosed with behavioral health disorders preferred.
- CCBHC knowledge and experience preferred.
- Knowledge of Trauma-Informed Care Practices preferred.
Must have reliable transportation and a valid driver’s license.
- Ability to assess individual needs, to coordinate services, and to communicate effectively.
- Knowledge of the social services system and the ability to connect individuals with appropriate services.
- Ability to write clearly and professionally on all services and internal communications.
- Ability to think clearly and logically while dealing with potentially dangerous and/or emotional situations.
- Ability to respond to individuals with patience, empathy and understanding.
- Organized, detail-oriented, and computer-proficient.
- Ability to demonstrate strong multi-cultural competency.
Bilingual (English/Spanish) preferred.
