What are the responsibilities and job description for the PASRR Habilitation Coordinator position at Collin County Mental Health Retardation Center?
Work is performed under the general supervision of the Program Administrator with broad latitude for the use of initiative and independent judgment. Responsibilities include: serving as the primary IDD point of contact for screening, intake, review of services statewide, referral and interest/wait list management. Work with individuals, families and stakeholders to assure understanding of the Texas system of IDD supports, gather eligibility information and support eligibility determination. For those individuals not immediately eligible work with interest/waiting list management and assure individuals are assigned to the appropriate local authority. Requires timely and thorough documentation, time management and organization skills, effective scheduling, adherence to crisis procedures, be flexible to assist with coverage as needed and effective communication with individuals, stakeholders, and supervisory staff.
Additionally:
PASRR Assessment In coordination with other team members monitor the HHSC PASRR Portal 24/7/365. Respond to alerts or requests from individuals in the community for PASRR NF placement. (All PL1 & PE notes entered within 7 days of alert)
Participate in the initial NF IDT/SPT meeting for individuals entering the NF and Annually.
Schedule and facilitate a Service Planning Team meeting (SPT) with NF staff, all providers, and key individuals in the persons life (LAR/Involved family members.)
Complete the individuals Service Plans in a person centered manner based on input/information gathered from the individual, those who know them best and the SPT.
Perform the Community Living Options (CLO) Process.
Timely document all individual related activities; Contact notes will be completed in a timely manner.
Serve as back up & support for other LIDDA FD functions:
o Screening eligibility & assessment
o Intake
o Benefit eligibility
o Waiver enrollment
o Diversion/Crisis/SSLC Application
o ECC
o Transition/COS
o Permanency Planning
o Interest list update contacts and documentation.
o Other duties as assigned.
Provide Monthly face to face contact.
To comprehensively monitor all ISP Outcomes and NF Care Plan supports at Quarterly SPT.
Support Transition /Enhanced Community Coordination (ECC)
- Bachelors degree in social work, psychology, or a related field from a four year college of university.
- Have at least one year of experience working with individuals with intellectual or developmental disabilities.
- Bi-lingual (Spanish) is a plus.
- Must have reliable transportation and a valid drivers license.
