The Social Worker is responsible for promoting the psychosocial and well-being of Collington residents by providing case management and other related support in collaboration with the Director of Social Services, in all levels of care, enabling continuity of care for residents and families and meeting regulatory requirements for licensed nursing beds in the health center. The Social Worker is also responsible for continuity of care to all residents for whom he/she is the primary designee.
Essential Functions
- Assesses residents’ needs and provide case management and support to promote and maintain optimal quality of life and well-being. Assesses changes in resident’s psychosocial health and assists residents with transition and brief crisis intervention. Supports aging in place at all levels of the continuum; provides timely assessments, counseling, education, support services, and referral. Meets with families as needed.
- Assesses residents' psychosocial needs utilizing approved assessment tools as needed.
- Coordinates care plans, assesses changes in resident's psychosocial health and assists resident with transitions and crises.
- Completes appropriate documentation upon admission, transfer, and discharge.
- May communicate with residents and families and assist with finding appropriate resources within the community.
- Communicates with Interdisciplinary Team regarding admissions, transfers, and discharges, scheduled family meetings, and other issues of common concern.
- Advocates for Residents Rights including abuse, neglect, exploitation and/or psychosocial/spiritual needs.
- Coaches, supports, and enables other team members to respond to resident’s emotional and/or psychological needs.
- Other duties as assigned.

Salary: Up to $90,000 based upon experience
