The Case Manager is responsible for delivering comprehensive care management services and providing community-based care coordination to address service gaps experienced by the unsheltered community. The Case Manager will work with a diverse population, ensuring culturally competent service delivery and promoting optimal care outcomes. ESSENTIAL DUTIES AND RESPONSIBILITIES

The following duties are considered essential to the role. The Case Manager must be able to perform each essential function satisfactorily:

Direct Care and Case Management

  • Provide services to all participants in a culturally competent manner, without regard to race, color, creed, national origin, marital status, sexual orientation, age, sex, religion, disability, or payment status.
  • Conduct outreach efforts to engage and enroll eligible participants, ensuring all documentation is accurate and up to date.
  • Conduct comprehensive assessments to identify participants' medical, behavioral health, and social needs, establishing personalized care plans.
  • Develop and provide education to participants on self-management techniques and address barriers to care (e.g., transportation, housing, access to healthcare).
  • Monitor and track participant progress toward achieving care plan goals, offering continuous support and guidance as needed.
  • Inform participants about their personal rights, responsibilities, and the organization's grievance procedures. Ensure all services are provided in a manner that supports participants' freedom of choice.
  • Work collaboratively with interdisciplinary teams (e.g., medical, behavioral health, and social services) to ensure coordinated care and improve outcomes.
  • Ensure adherence to ethical and professional standards of practice, maintaining participant confidentiality and respecting cultural diversity.
  • Provide participants, families, and the community with information on healthcare issues including prevention, early detection, and remedial care.
  • Ensure provisions are in place for participants with physical challenges, supporting their safety and well-being.
  • As needed, assist with transportation to appointments, outings, and other essential activities.

Teamwork and Collaboration

  • Maintain professional, cooperative working relationships with internal and external team members and stakeholders.
  • Collaborate effectively to meet program outcome expectations as set by CCAH and other funding agencies.
  • Actively participate in regular team meetings, case reviews, and training sessions to ensure optimal care and outcomes.
  • Provide input to management regarding program improvements and assist in monitoring program effectiveness and participant satisfaction.

Reporting and Documentation

  • Report any suspected abuse of dependent adults and elders directly to Law Enforcement, Adult Protective Services, the Ombudsman, or Community Care Licensing, in accordance with mandated reporting laws.
  • Maintain clear, accurate, and professional records, adhering to all documentation requirements.
  • Manage caseload expectations and work collaboratively to ensure all program requirements are met.
  • Submit accurate and timely invoices, billing data, and other reports as required.

QUALIFICATIONS

Education and/or Experience

  • Bachelor’s degree in Psychology, Social Work, or a related field OR three years of experience in an outreach program providing direct services to individuals with housing struggles or related needs.

Knowledge

  • Familiarity with HIPAA, OSHA, state, and local regulations, particularly those set by the Department of Health Services.
  • Comprehensive understanding of trauma-informed care and its implementation in working with vulnerable populations.

Language Skills

  • Ability to read, speak, write, and understand English fluently enough to follow and communicate regulatory requirements, policies, and participant care instructions.
  • Effective presentation skills, with the ability to respond to questions from participants and interdisciplinary team members.

Other Skills and Abilities

  • Ability to prioritize tasks, meet deadlines, problem-solve, and adapt to changing priorities.
  • Strong interpersonal skills with the ability to maintain positive working relationships.
  • Proficient in computer applications such as email, word processing, and case management software.

Additional Requirements

  • Must be at least 21 years of age and accepted by the organization’s vehicle insurance provider.
  • Cleared by the Department of Justice and relevant county background checks.
  • Must complete all required annual training as mandated by Santa Cruz County and the organization’s policies.

Job Type: Full-time

Pay: $28.00 - $30.00 per hour

Expected hours: 40 per week
