Job Purpose:
We are looking for a passionate RN Resident Care Coordinator who is energetic and team-orientated to assist with the day-to-day management of the nursing department by collaborating with the Director of Nursing to plan, organize, and direct the Nursing Service of Colonial Manor Nursing Home. The RN Resident Care Coordinator works under the general supervision of the Director of Nursing, assumes responsibility for the Nursing Department in the absence of the Director of Nursing, and, at times, serves as the charge nurse, the RN on-call, or the on-site person in charge of Colonial Manor. Evaluate and promote total resident care (physical, emotional, psychological, social, and spiritual). Participates with all other departments in planning and implementing the nursing component of care and assists in the evaluation processes. Coordinates team education and endorses the team concept of providing care through the various disciplines and assists in supervising the day-to-day function of the facility including the delivery of nursing care, and the nursing actions of employees: RNs, LPNs, TMA’s, and CNA’s.

Qualifications
- Current RN License
- Strong knowledge of managed care and patient assessment
- Experience in long term care setting
- Proficient in discharge planning and understanding of HIPAA regulations
- Familiarity with medical terminology and physiology
- Background in inpatient and post-acute care, with an emphasis on patient service and care
Job Type: Full-time
Pay: $35.00 - $44.00 per hour
Expected hours: 40 per week
