Colonial Poplin Nursing Home in Fremont, NH is looking for a per diem Occupational Therapist. The Director of Rehab has been an employee of Colonial Poplin for almost 20 years and has a great team of PTs, PTAs, OTs, OTAs and STs. She collaborates with the Rehab Team and works alongside the Rehab staff to provide care to our skilled, long-term and assisted living residents. We are looking for a motivated, energentic person to join our team.
OT ESSENTIAL DUTIES AND RESPONSIBILITIES
Conducts resident screens (without interpretation) as allowed by state practice act.
Helps residents with rehabilitative activities and exercises outlined in a treatment plan developed in collaboration.
Monitors an individual’s activities to make sure they are performed correctly and to provide encouragement.
Records resident’s progress.
Provides training to residents, caregivers and families according to the resident’s care plan.
Completes required forms and documents in accordance with company policy and state and / or federal regulations.
