Overview
We are seeking a dedicated and detail-oriented MDS Coordinator to join our healthcare team. The ideal candidate will play a crucial role in ensuring compliance with Medicare regulations and standards while providing exceptional care to our patients. This position requires a strong understanding of the MDS process, documentation review, and utilization review within various healthcare settings, including home care and outpatient services. The MDS Coordinator will work collaboratively with medical staff to enhance patient outcomes and streamline operations.
Responsibilities
- Conduct comprehensive assessments and documentation reviews for patients in accordance with Medicare guidelines.
- Oversee the completion of the Minimum Data Set (MDS) assessments and ensure timely submission.
- Collaborate with interdisciplinary teams to develop individualized care plans based on assessment findings.
- Perform utilization reviews to evaluate the appropriateness of services provided to patients.
- Maintain accurate records within eClinicalWorks and other relevant systems.
- Provide training and support to staff regarding MDS processes and documentation requirements.
- Monitor compliance with regulatory standards and implement corrective actions as necessary.
- Facilitate communication between patients, families, and healthcare providers to ensure continuity of care.
Experience
- Previous experience in a medical office or healthcare setting is essential, with a focus on home care or outpatient services preferred.
- Proficiency in eClinicalWorks or similar electronic health record systems is highly desirable.
- Strong knowledge of Medicare regulations, MDS processes, and documentation standards is required.
- Experience in pediatrics is a plus but not mandatory.
- Excellent organizational skills, attention to detail, and the ability to manage multiple priorities effectively.
