What you will do:
We are looking for a CARE MANAGEMENT COORDINATOR who can help shape our vision and support our mission. Here is what the position will look like.
Manages inbound and outbound calls demonstrating a customer-focused philosophy for all customers including, but not limited to, providers, members, potential members, state agencies and other internal and external customers.
Screens potential clients to determine the appropriateness of and eligibility for services and assists clients in making informed choices regarding care management.
Explains and discusses the role of the care manager program parameters, and client rights and responsibilities to proceed with the structured assessment process.
Establishes effective collaboration, communication, and coordination among all responsible parties of an individual’s interdisciplinary health care team.
Utilizes resource materials, policies and procedures, handouts, databases, and training opportunities to provide information, identification and enrollment to providers (primary care and specialty) education, and referrals for all members.
Communicates with clients utilizing person centered thinking and language through all aspects of care coordination.
Responsible for accurate and timely completion of all forms, reports, and documentation of care management activities.
Maintains professional and ethical manner with all interactions and meets performance, quality, customer service, and coordination standards as assigned by the department management team.
Assists clients and providers in understanding the complaint, grievance, and appeal process.
Participates in training and staff development opportunities. Actively participates in team meetings and communicates progress and barriers with Coordinator Supervisor and/or Department Manager.
What you will bring:
Education: High school diploma or equivalent required. Associates degree in health, social services or additional college courses preferred.
Experience: Customer service or call center experience within the healthcare industry preferred.
Knowledge, Skills, and Abilities: Ability to work within a call center environment and answer inquiries in a professional manner. Knowledge of local health and community agencies, systems of care, health insurance, supplemental insurance, and long-term care insurance preferred. Demonstrates support for the company’s mission, vision and values. Position requires strong written and verbal communication skills, organizational and time management skills and ability to handle multiple priorities. Must have strong computer skills, ability to work with Microsoft Office products and navigate multiple databases and electronic records. Requires the ability to use the complaint, grievance and appeal process and procedures to facilitate quality outcomes and or resolutions for the benefit of the members, providers and Colorado Access. May be required to manage multiple priorities and projects with tight deadlines.
Licenses/Certifications: A valid driver's license and proof of current auto insurance will be required for any position requiring driving.

Pay, Perks and Benefits at Colorado Access:
The compensation for this position is $52,820.00 - $58,450.00 annually and is a non-exempt (hourly) position. Colorado Access has provided a compensation range that represents its good faith estimate of what Colorado Access may pay for the position at the time of posting. Colorado Access may ultimately pay more or less than the posted compensation range. The salary offered to the selected candidate will be determined based on factors such as the qualifications of the selected candidate, departmental budget availability, internal salary equity considerations, and available market information, but not based on a candidate’s sex or any other protected status.

Where you will work:
This position will be a hybrid model work environment, a blend of ‘In-Office’ and ‘Remote.’
