Patient Care Coordinator
The role of the Patient Care Coordinator is to serve as a liaison between the clinical staff and the patient while the patient is under the care of CCRM and to ensure that there are no gaps in the patient's experiences, clinical care, or administrative needs.
The Patient Care Coordinator demonstrates knowledge and skills in the care of their fertility patients. The Patient Care Coordinator works with the Nurse Coordinators to facilitate care for the fertility patient. This position reports directly to the Nurse Manager and takes daily direction from the Nurse Coordinators / Physician / Nurse Manager and Assistant Nurse Manager.

ESSENTIAL FUNCTIONS
- Meet with new patients along with the nurse.
- Provide next step instructions to the patient regarding workup items ordered by the physician after the initial physician consult. If immediate cycling is anticipated - will refer to primary RN. Will initiate workup checklist and chart audit and sign out to RN when patient has been established and treatment plan is finalized.
- Track and audit all patient consent forms through completion. Alert RN if deficiencies are noted.
- Track patient workup status to ensure current pre-conceptual labs, communicable testing, pap smear, annual exam, and mammogram (if applicable) have been completed. Follow up with patient regarding any missing consents / medical reports / lab workup.
- Send abnormal labs to RN to review with physician.
- Send all normal preconception lab results to patient via secure portal.
- Transcribe, enter and verify data from source documents into the electronic medical record in an accurate and timely manner.
- Fax patient prescriptions and prescription refills as directed by RN.
- Perform chart audits for weekly ART meeting, confirming with RN that all treatment care plans are complete and accurate.
- Monitor nurse team flags for nurses on team and facilitate flag completion as needed.
- Answer patient calls and emails that are within scope of practice and document action taken in patient EMR.
- Work with Nursing Leadership to act as a liaison for difficult situations and patient issues.
- Route patient questions, problems, complaints, and concerns to appropriate personnel.
- Oversight of processes, implementation of processes to ensure that the ART process runs smoothly for patients.
- Other duties as assigned.

This job operates in a professional office and clinical setting. This role routinely uses standard office equipment such as computers, including computer keyboards and mice; telephones and photocopiers; scanners; filing cabinets.
While performing the duties of this job, the employee is regularly required to communicate, see and hear. The employee is frequently required to sit; will occasionally stand and / or walk; use hands and fingers to grasp, pick, pinch, type; and reach with hands and arms. Employee is required to have close visual acuity to perform an activity such as : preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading; operation of standard office machines and equipment.
