The Colorado Coalition for the Homeless currently seeks a Bilingual Early Intervention Coordinator to oversee and maintain all services for the Early Intervention Services Program and provides services that promote the healthy physical, emotional, educational, and social growth of children experiencing homelessness, prenatal to 6 years of age. The program acts as a link between children/families and needed services. The Early Intervention Services Coordinator promotes the developmental growth and social-emotional progress of children and ensures that they receive the skills and preparation needed to enter school ready to learn. This position also provides support services to children and families using trauma-informed care and promotes equity through community engagement. This position is located at 2111 Champa St., Denver, CO 80205 and reports to the Program Manager. Essential Job Functions  Responds to all referrals in a timely manner and consults with referral source as needed to further discuss the needs of the family. Follows up on referrals by reaching out to parent/caretaker to discuss program and concerns. This may include providing outreach services to families experiencing homelessness in parks, motels, shelters, encampments, and transitional housing. Identifies needs of children through observation, screenings, evaluations, and parent/caretaker consultation to determine next steps. Ensures that families have a clear understanding of the program.  Completes comprehensive assessment and case plan with all families. Refers families to appropriate programs within the community (such as Community Centered Boards for early intervention services, Head Start centers, pediatric medical providers, school district Child Find programs, etc.) Develops and maintains a relationship with parents and children through regular face-to-face visits and phone contacts. Develops understanding of barriers to services and resources that families face. Work directly with families and community partners to establish services and ensure continuity of care. Completes Electronic Health Record documentation in a timely and accurate manner. Maintains compliance with agency and HIPAA standards, including completing Releases of Information and registration paperwork. Assist in coordination of Healthy Kids Fair at Stout Street Health Center. Initiate and maintain relationships with community partner agencies to promote coordination of care. Initiate and maintain relationships with other departments within CCH that work with families eligible for Early Intervention program, including Stout Street Health Center and Department of Outreach & Engagement Services. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.  Qualifications Summary Master’s degree in mental health, social work or related field or Bachelors degree with equivalent experience required. Previous experience in implementing programming for young children and families who are homeless or at-risk of becoming homeless, knowledge of Medicaid and CHP enrollment procedures, and knowledge of early childhood education/development preferred.
