This role involves working across multiple locations, assisting community members with housing applications, facilitating pre- and post-move tasks, and ensuring a seamless transition to housing stabilization services.
Schedule / Hours
The position requires the ability to perform the essential functions with or without reasonable accommodation. Tasks may involve prolonged periods of standing, sitting, or walking, as well as lifting and carrying objects up to 25 lbs. Additional requirements include occasional reaching, bending, crouching, or kneeling, along with manual dexterity to operate tools, equipment, and technology. Schedule / Hours
- 40 hours per week with flexibility for occasional evenings and weekends.
- Includes time spent in community member-facing work, administrative tasks, transportation, and collaboration with HCC and other stakeholders.
- Serve as the first point of contact for community members, providing a welcoming and supportive environment. Meet with community members at their assigned locations as directed by Housing Central Command.
- Conduct Pre-Housing Navigation over a 30-day period with individuals referred through HCC.
- Work with community members to complete all required housing applications for units identified by the HCC Unit Team.
- Assist community members with acquiring necessary documentation (e.g., IDs, income verification) to support housing readiness.
- Transport community members as needed for pre-move-in and move-in tasks, including unit tours, landlord meetings, and moving personal belongings.
- Attend two daily meetings with the HCC Administration Team for updates and coordination.
- Schedule and coordinate move-ins, including furniture deliveries, in collaboration with the HCC Administration Team.
- Collaborate with community members and landlords to finalize housing placements and address any pre-move concerns.
- Facilitate a smooth hand-off to housing stabilizers for the community member’s 12-month housing stabilization period.
- Enroll community members into HMIS and maintain accurate and up-to-date records, including case notes and community member documentation, in compliance with data standards.
- Ensure proper documentation of all activities, communications, and community member progress.
- Assist in the preparation of reports for HCC and other stakeholders as required.
- Provide excellent internal and external customer service, maintaining a professional and supportive demeanor.
- Actively participate in team meetings, training sessions, and supervision, contributing to a culture of collaboration and accountability.
- Work collaboratively with the HCC Unit Team, landlords, and community partners to support community member housing goals.
- Promote an anti-racist, trauma-informed, and culturally competent approach in all interactions.
- Support other housing-related tasks as needed to address community member or program needs.
- Adapt to the dynamic nature of the role and participate in special projects or assignments as directed.

Requirements
Qualifications:
- High School Diploma or equivalent and one year either working with unhoused individuals, in non-profit. Additional education or experience in social work, human services, or related fields preferred.
- Experience working with individuals experiencing homelessness or housing insecurity strongly preferred.
- Familiarity with HMIS or similar data systems is a plus.
- Reliable transportation is required.
- Cell phone to download and access CVC applications is required
