Columbia Community Mental Health is a nonprofit agency and Certified Community Behavioral Health Clinic that provides services to individuals with mental, addiction and developmental disabilities. We are looking for a qualified Care Coordinator to join our team!

Position : Care Coordinator

Status : Non-exempt

Shift : Mon-Fri 8 : 00 am to 5 : 00 pm

Salary Range : $56,804.80 annually - $73,444.80 annually. We use a bona fide factor system to determine starting wage (years of experience, education, credentials, and specialty skills that relate to the job requirements).

  • Position Requirements : requires a QMHA : a bachelor's degree in a behavioral science field from an accredited college or university or three years of full-time professional experience working in the mental health related field. Note : All QMHA positions in the state of Oregon will need to be filled by candidates who are officially QMHA certified or registered with MHACBO. For details, go directly to MHACBO (www.mhacbo.org)

    Summary : The Wraparound Care Coordinator is responsible for coordinating / supporting behavioral health care within CCMH and the referrals to clinically indicated services and supports outside of the agency. Services include but are not limited to case management, skills training, psychoeducation, care coordination, and advocacy. Group and individual treatment modalities are utilized.

    Working conditions : This position works in an office environment and in the community. This position requires driving and meeting clients where they are located, their home, and in the community.
