The Community Developmental Disabilities Program (CCDP) provides access to local, regional, and statewide resources for children and adults who experience a developmental disability. In addition to comprehensive case management and service coordination, our staff assist with early Intervention and phase of life transitions. Our team will work closely with eligible individuals to ensure needs are addressed in a comprehensive and collaborative manner.

Columbia Community Mental Health (CCMH) is a nonprofit agency and Certified Community Behavioral Health Clinic that provides services to individuals with mental, addiction and developmental disabilities. Position : Service Coordinator

Department : Intellectual & Developmental Disabilities (I / DD)

Shift : Full time (Monday – Friday, 8 : 00am to 5 : 00pm)

Salary Range : $59,155.20 - $75,857.60 (annual) We use a bona fide factor system to determine starting wage (years of experience, education, credentials, and specialty skills that relate to the job requirements).

  • Position Requirements : Bachelor's Degree and one-year relevant experience or three years in human services with at least one year working with DD clients required. Case management experience in Oregon preferred. Combination of education / experience that demonstrates the ability to perform the duties outlined for this position may be considered.

    Summary : Service Coordinators empowers individuals with intellectual and developmental disabilities, mental health challenges and other social barriers to lead self-determined lives and reach their full potential! As a Service Coordinator, you'll be responsible for implementing and monitoring systems that support the IDD community. You'll facilitate health / safety, enhance people's quality of life, help them lead their desired lifestyles and gain increased levels of independence, productivity, and social / physical integration.

    Working conditions : This position works in an office environment as well as in the community. This position requires driving to different locations within Columbia County and some occasional driving to locations elsewhere in the Portland Metro. The Service Coordinator will also be available to provide services in the client's residence as needed.
