Columbia River Home Health is seeking an experienced, full-time Home Health Community Liaison to cover the Tri-Cities, WA area! We are looking for someone just as passionate as we are about providing quality patient care.

Job Summary:
- Assist patients in the process of navigating post-acute care
- Assess, coordinate, and evaluate options with a primary goal of providing a safe transition from acute care to home
- Develop transition plans that are patient-centered, promoting quality and efficiency in the delivery of post-acute care
- Represent Columbia River in transitional care activities and strategic relationships with health systems, hospitals, inpatient facilities, and physician groups
- Monitor ongoing quality assurance visits with referral sources
- Meet and/or exceed referral and admission goals
- Positively impact patient outcomes and referral source satisfaction

Qualifications:
- Must have community marketing/liaison experience with home health, hospital, and other healthcare industry contacts
- Demonstrates autonomy, organization, and cooperation
- Excellent communication skills and the ability to build relationships, interact well with diverse individuals, and have strong presentation skills
- Be able to work independently throughout the community with 85% of your time being spent out in the field
