JOB SUMMARY
The Intake Coordinator is responsible for initiating and coordinating Home Health referrals, gathering all
necessary referral and Payor information and then inputting into EMR, verifying patient information and initial HH eligibility with patient and referral source, and coordinating and scheduling date for HH admission. This
position ensures that information obtained at time of referral is accurate and complete. This position represents Home Health to customers and patients effectively promotes Home Services and products.
DUTIES & RESPONSIBILITIES
1. Coordinate and process incoming referrals from Home Services Liaisons.
2. Coordinate and ensure insurance information is obtained and accurately recorded into electronic medical record (EMR).
3. Coordinate and evaluate patient information for referrals within guidelines
4. Accurately documents referral information utilizing critical thinking skills
5. Be responsible for triaging and coordinating with appropriate team members for review if
incomplete.
6. Serve as a liaison between referral sources, MD office, patients / families, department supervisors,
managers, insurance team, Access RNs to ensure timely patient care.
7. Coordinate and communicates with appropriate team members to ensure service standards are
met.
8. Oversee all fax communications.
9. Be responsible for coordinating accurate insurance information and problem solves insurance
eligibility with insurance team.
10. Utilize critical thinking skills to coordinate and assign appropriate start of care date under clinical guidelines utilizing clinical staff as appropriate.
11. Coordinate with referral sources, inclusive of hospital, physician, facility customers and insurance case managers to obtain additional information, confirm referral receipt and communicate start of
care date.
12. Prepare specific reports as directed for distribution within Home Health and other departments.
13. Be responsible for staying current on CMS regulations and maintaining current knowledge of Home Services products and services, and other referral sources.
14. Seek at all times to enhance Home Services Departments relationship with referral and
reimbursement sources.
15. Able to use critical thinking to multitask and prioritize work.

JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)
1. High School Diploma and associate's degree.
2. Educational coursework in medical terminology -OR- equivalent education / experience; prefer
medical office experience. A nursing background is preferred but not required.
3. Two (2) years of experience in customer service including demonstrated computer proficiency.
4. Demonstrates autonomy, assertiveness, flexibility and cooperation in performing job
responsibilities.
