Position Description:
The primary focus of this position will be a Lead Referral Coordinator within a housing first model and will focus on the Trueblood populations with referrals originating from multiple sources. The Lead Referral Coordinator will develop relationships with the FHARPS Team, FPATH, OCRP, Forensic Navigators and Diversion Navigators, as well as any other community partner that may make referrals to the FHARPS Program. The LRC will coordinate the referrals of the housing team. The LRC will assist to provide leadership and training, coaching, and oversight of daily tasks and work closely with the Manager and Director. The LRC will also have administrative duties that include facilitating team meetings I the Managers absence, coordinating with community partners and organizations, and monthly and quarterly reporting as needed.
Essential Job Functions:
- Familiar with Landlord Tenant Law and ensure that CRMHS follows applicable laws for housing programs.
- Familiar with location matching, roommate matching, background checks, leasing/application packets
- Familiarity with Rent/Lease information and assistance (e.g. notices, evictions, housing support, rent collection)
- Coordinate with community partners and organizations for referrals, housing placements, braided funding and long term supports
- Facilitate consumer groups as needed
- Facilitate Team Meetings
- Provide all aspects of Housing Support Services to individuals enrolled through duties assigned
- Act as Liaison between FHARPS consumer and global leasing programs staff.
- Coordinate with local profit and non-profits (e.g. HEN, VHA, Home and Community Services, Clark County Jail, SUD, Crisis,CFTH and MHC, etc) as needed
- Participate in weekly and monthly meetings as applicable
- Work as a team member with other CRMHS staff and other agencies when appropriate
- Coordinate care internally to assure timely communication relative to consumer needs and preferences
- Maintain all appropriate records and charts
- Provide staff training and follow up
- Work with a team of up to 10 Peers and/or Case Managers
Non-essential Job Functions:
- Maintain communication and cooperative working relationships both within the agency and community at large.
- Other duties as assigned by Program Director and Program Manager.
Education, Licensing and Experience:
- High School Diploma or GED
