Starting Salary Range –
100,000 annually depending on experience.
Summary :
As opposed to traditional primary care provider roles, the role of this position is to provide physical health care services to agency clients whom are experiencing a gap in their access to traditional primary care services. Care delivered within this position can include basic primary care visits, medical screenings, urgent care needs, wound care, chronic health condition management, hospital follow ups, and meeting gap mental health needs managed within the typical scope of a primary care practice.
The responsibilities include initial and updated medical evaluations, health screenings, routine follow-up care and medication management for chronic medical conditions including hypertension, heart disease, diabetes and medical complications of psychiatric medications. Ensure timely and quality delivery of medical reviews, and support staff clinically and administratively. Focus populations include individuals with mental health conditions, substance use disorders, individuals with Medicaid / Medicare, the uninsured and underinsured, as well as individuals experiencing homelessness or other severe social determinants of health. The GAP Medicine provider builds a bridge back to traditional primary care services and office visit volumes are often lower than traditional practices while having dramatic impacts on patient lives.
Essential Job Responsibilities :
- Works in an independent and interdependent relationship with members of the medical staff and other providers that allows for consultation, collaboration, or referral.
- Prescribes medications, individualized treatments, and ongoing medication management.
- Orders lab and diagnostic tests when appropriate and interprets test results.
- Maintains legible, accurate, and confidential medical records.
- Completes documentation within 24 hours of patient encounter.
- Coordination of care with other providers, counselors and community care providers.
- Coordinator medical needs with outside care teams and other medical providers.
- Provide medical support to the Mobile Team, seeing agency clients in the field on an as-needed basis and providing medical services at ‘pop up’ clinics established in areas of high need. Maintain communication and cooperative working relationship both within the agency and community at large.
- Projects a positive and professional image at all times.
- Provide non-managerial leadership within teams assigned, providing work direction to Medical Assistants and Nursing as necessary, and other team members as appropriate.
