Starting Pay Range –
LPN - $31.72 to $34.29 /hour
RN - $43.27 - $47.02 /hour
Summary:
The Nurse Care Coordinator (NCC) is a nurse with a valid Washington State license as a registered nurse or licensed practical nurse working in an office-based opioid treatment, providing skilled nursing services to opioid dependent clients. Treatment includes, but is not limited to intake, assessment, evaluation, admission, triage, medication induction and stabilization, teaching, and monitoring.
Essential Job Responsibilities:
- The Nurse Care Coordinator (NCC) will assist physician and nurse practitioner in managing clients with Opioid Use Disorder (OUD) in Medication Assisted Treatment (MAT) program at multiple locations including the main office and the Clark County Jail
- NCC will provide first contact with potential MAT clients, including taking prospective client phone calls, providing program and medication information, answering questions and scheduling induction appointments.
- NCC will provide laboratory slips as ordered by the provider and/or complete blood draws, obtain medical history and address concerns, complete consents to treatment and other intake paperwork with clients, monitoring inductees, and schedule follow up appointments for medication monitoring and recovery support
- NCC will manage day-to-day care of clients, supporting their recovery from OUD and the complex biopsychosocial problems that often accompany OUD.
- NCC will administer medication per agency/state/federal regulations
- NCC will provide education surrounding infectious diseases, including hepatitis C. Schedule initial appointments with provider and nurse, and provide hepatitis C medication education and treatment options to clients
- In conjunction with counseling staff, NCC will work to encourage retention and success in treatment by identifying biopsychosocial problems which may inhibit recovery progress and identifying/implementing client-driven solutions to these problems
- NCC will assist in providing staff education surrounding MAT treatment
- NCC will work collaboratively with community partners and ensure effective communication with outside partners is utilized to provide client care
- NCC will assist the STR grant manager in meeting grant funding goals
- NCC will attend staff meetings as required
- NCC will attend and actively participate in Division of Behavioral Health & Recovery (DBHR) identified or sponsored trainings and conference call meetings as part of statewide Nurse Care Manager program
- Meet as needed (phone or in-person) with STR grant manager and DBHR Contract Manager to discuss performance-based contract requirements, compliance, and problem-solving
- Participate in the use of a certified electronic health record system.
- Ensure the use of Evidence-Based Practices (EBP) for all screening, assessment, and interventions.
- Assist in developing nursing billing protocols and MAT services.
- Serve patients in a culturally competent, patient centered manner and ensure activities adhere to Culturally and Linguistically Appropriate Services (CLAS).
- Establish and implement a plan to mitigate the risk of diversion and, as appropriate, ensure the appropriate use of MAT by patients.
- Promote abstinence from tobacco products (except with regard to accepted tribal traditional practices) and to integrate tobacco cessation strategies and services.
- Assist in collecting data per performance-based requirements and submit to DBHR within seven (7) calendar days of data collection through the DBHR provided Common Data Platform or Patient Log.
- Maintain all paperwork/charting requirements as established by laws or agency contracts
Non-essential Job Functions:

Education, Licensing and Experience:
- A minimum of 2 years full-time experience in direct treatment of persons enrolled in a medication-assisted treatment program.
- Valid Washington State license as a registered nurse or licensed practical nurse required.
