Position Summary
The Clinical Coordinator III, Nurse Practitioner will function in the outpatient setting of the Thoracic Surgery Division. Collaborating with Thoracic Surgery/ Price Center multidisciplinary team to care for and manage pre-operative and post-operative patients.
Responsibilities
- Coordinates medical and surgical management of Thoracic surgery patients in the outpatient setting.
- Assisting Thoracic surgeons in the outpatient clinic to perform comprehensive history and physical exams, develop differential diagnoses, implement plan of care and document patient’s response to care.
- Conduct pre-operative history and physical exams, as well as obtain surgical consents.
- Provide direct patient care by evaluating patients awaiting kidney transplant, as well as post kidney transplant recipients, and kidney donors.
- Collect, order and interpret lab work and diagnostic studies under agreed upon protocols.
- Order and obtain prior authorizations for medications.
- Ensure physicians meet meaningful use metrics.
- Develop practice protocols, standard of care, and patient education material for Thoracic surgical patients.
- In conjunction with the research team, monitor and provide care to patients participating in research studies, as needed.
- In partnership with Quality, Research and Education group, help develop and manage detail clinical databases to evaluate and improve patient outcomes.
- Attend professional meetings and conferences, maintain professional affiliations and enhances professional development to stay current with trends in health care administration.
- Support other functions as assigned.
Minimum education:
- Nurse Practitioners: Master of Nursing
- Requires bachelor's degree or equivalent in education and experience, plus five years of related experience.
- Advanced Practice Registered Nurse is either Masters or Doctorate prepared as a Nurse Practitioner, as well as board certified with ANCC or AANP. The Nurse Practitioner must have a valid license as a Registered Nurse and as a Nurse Practitioner
- Current, or be eligible for, a DEA license for New York State.
- BLS Certified.
- Intermediate MS Office skills Must be able to successfully complete systems training requirements.
- Masters preferred.
- ACLS preferred.
Qualifications for Candidates include but are not limited to the following:
- Ability to perform comprehensive history and physical examination.
- Ability to manage and coordinate a patient caseload.
- Work within a collaborative agreement/protocol with a physician and health care team.
- Demonstrate leadership ability to manage patients and interact effectively with peers and physicians.
