What are the responsibilities and job description for the Quality Navigator I position at Columbia Valley Community Health?
Overview
The Quality Navigator (QN) will work to improve the health of our patient population by collaborating with the primary care and population health teams. The primary focus is to assist the clinic in obtaining quality measures through data scrubbing, validation, and patient outreach. The Quality Navigator I is an integral part of the care team, supporting both patients and providers to achieve excellent patient care and quality scores.
Job Specific Competencies.
1. Assists Population Health and Medical Teams with obtaining quality measures.
2. Assists Population Health and Medical Teams with the management of the Quality tab in the electronic health record, Athena.
3. Assists with coaching and training of teams as needed.
Qualifications
1. Education: Associates Degree or equivalent work experience.
2. Certification/Licensure: N/A
3. Experience: Two years’ experience in Community/Migrant Health, or Public Health preferred.
4. Language Skills: Bilingual in English/Spanish preferred.
Job Type: Full-time
Pay: $23.00 - $31.52 per hour
Expected hours: 40 per week
