Columbia Valley Community Health (CVCH) is growing! CVCH is a dynamic, progressive community health center with locations in Wenatchee, East Wenatchee, and Chelan. We have an excellent opportunity for a Quality Navigator at our Wenatchee location.
The Quality Navigator (QN) will work to improve the health of our patient population by collaborating with the primary care and population health teams. The primary focus is to assist the clinic in obtaining quality measures through data scrubbing, validation, and patient outreach. The Quality Navigator I is an integral part of the care team, supporting both patients and providers to achieve excellent patient care and quality scores.
Position Requirements:
· Education: Associates Degree or equivalent work experience.
· Certification/Licensure: N/A
· Experience: Two years’ experience in Community/Migrant Health, or Public Health preferred.
· Language Skills: Bilingual in English/Spanish preferred.
