Overview
The Case Management Officer is intended to support identified individuals who have a history of being convicted of felony crimes coupled with a need for substance use treatment and/or at risk of re-offending. The Case Management Officer will develop, implement, coordinate, and evaluate an "Individual Case Plan" (ICP) with participants in the Columbiana County Court of Common Pleas Day Reporting Program (CCDRP) as well as felony offenders who are under community control. This position will provide supervision and rehabilitative services to participants through assessment and referral. The Case Manager will work with the Columbiana County Court of Common Pleas Adult Probation Department and report to the Columbiana County Day Reporting Center Director.
Responsibilities
- Conduct comprehensive assessments of clients' needs and circumstances.
- Develop, implement, and monitor individualized service plans tailored to each client.
- Provide crisis management support to clients in need of immediate assistance.
- Collaborate with community organizations and resources to facilitate access to services such as workforce development, treatment, financial assistance, etc.
- Advocate for clients in navigating systems
- Maintain accurate and up-to-date case records and documentation.
- Offer mentoring support to clients to promote personal growth and self-sufficiency.
- Engage in ongoing training and professional development to enhance skills required for this position
- Participate in court proceedings as necessary
Requirements
- Bachelor’s degree a related field; relevant experience may be considered in lieu of a degree.
- Strong knowledge of community resources
- Excellent interpersonal skills with the ability to build rapport with diverse populations.
- Strong organizational skills with attention to detail in case documentation.
- Ability to work independently as well as collaboratively within a team environment.
- Proficient in using technology for case management software and documentation purposes.
- Valid driver's license and personal transportation
