What are the responsibilities and job description for the Therapy Professional position at Columbine Manor Care Center?
The successful candidate will be responsible for:
* Establishing, assessing, and modifying realistic, measurable, timely, and functional goals
* Overseeing and evaluating care given by SLP CFYs (SLP CCC only)
* Following speech therapy treatment plans for patients under direction of supervising SLP CCC and in accordance with Federal and State guidelines
* Charting appropriately and timely
* Utilizing therapy software accurately
We are looking for a highly qualified Speech Language Pathologist who possesses the following qualifications:
* Graduate degree in speech therapy (MS or MA SLP) from an accredited program
* ASHA certification (CCCs) or eligibility for Clinical Fellowship Year (CFY)
* Current licensure in applicable State
* One year experience in post-acute care or related setting preferred
