Purpose
Under the direction of the Director of Emergency Services, the Manager of Emergency Services Case Management service provides leadership and oversight to OEN and Shelter Case Managers by providing case management, outreach and diversion services to literally homeless adults in the Greater New Haven area. This position plays a key role in collaborating with government, local mental health and substance abuse service providers, housing providers, the GNH CAN Systems and other agency programs to serve the most complex and service disengaged groups.
Duties and Responsibilities
- Provide supervision, leadership and direction to all case management staff and Programs including OEN, Urban Initiative, PATH, How Can We Help, Veterans Affairs, New Beginnings, Shelter Case Management, and any other seasonal project with case management involvement.
- Conduct consumer orientations.
- Leads weekly rounds meetings for outreach team.
- On-Call responsibilities
- Acts as the Point in Time Count Coordinator
- Create an environment that is welcoming and provides an enriching experience for all service recipients.
- Ensures that services are carried out with fidelity to each practice.
- Integrates Outreach services into all aspects of Columbus House programs.
- Responsible for intake, service, and referral coordination for program clientele
- Monitors program budget and ensures adherence and fiscal integrity.
- Other related duties as assigned.
Qualifications
- Bachelor’s Degree with a minimum of two years’ experience working in a social service, mental health, or substance abuse agency in an administrative or management role (preferable).
- Ability to work to improve teams and foster teamwork.
- Excellent organizational and time management skills
- Strong command of MS Office software modules
- Strong written and oral communication skills
- Knowledge of Motivational Interviewing, Person-Centered-Planning, Harm Reduction, Trauma Informed and Gender Specific Care and Housing First philosophies and models.
- Experience serving persons with histories of mental health, substance abuse and homelessness.
- Valid drivers’ license with a good driving history and daily access to an automobile
Supervised by: Director of Emergency Services
Schedule: Monday- Tuesday 9am- 5pm & Wednesday & Friday 10am-6pm
Salary range: $58,656 to $62,000
