Purpose

Under the dual direction of the Director of Emergency Services and CAN Manager, CAN Diversion Specialist supports the mission of the Greater New Haven Coordinated Access Network Task Force members, public and private partners, and United Way team to develop, implement, and measure homeless services that address homelessness for chronically homeless adults, youth, and families. The CAN Diversion Specialist will work closely with the CAN Manager, CAN System Coordinators, Family Navigators, and community providers to assist individuals and families to access emergency shelters, implement a set of services to expedite the process of housing chronically homeless individuals , diversions and exit homelessness to a variety of housing options in the GNH CAN service area (Ansonia, Beacon Falls, Bethany, Branford, Derby, East Haven, Guilford, Hamden, Madison, Milford, New Haven, North Branford, North Haven, Orange, Oxford, Shelton, Seymour, West Haven, or Woodbridge).

Duties and Responsibilities

  • Assessment and Diversion (Approximately 90% of time)
  • Provide CAN assessments for clients scheduled through 211
  • Provide CAN assessment to clients from Hamden primarily.
  • The CAN Diversion Specialist will provide CAN assessments in person or over the phone, as needed.
  • The CAN Diversion Specialist will be station at the Annex Office at Columbus House. Sometimes the CAN Diversion Specialist will need to conduct CAN Assessments at Hamden Town Hall office.
  • Attempt to divert client away from shelter using and all resources available (Diversion funding or local community resources.)
  • Record all outcomes in HMIS and smart sheets.
  • Build and maintain trusted relationships with frontline staff, management and leadership from CAN participating agencies.
  • Respond to questions from agency staff / clients about CAN assessment status, shelter waitlist status and general CAN Entry system
  • Communicate effectively with 211
  • Assist with generating reports related to overall system functioning and the number of clients added and removed from shelter waitlist, diverted, and enrolled in shelter.
  • Track and report on follow up for clients diverted at CAN assessment
  • Maintain regional shelter waitlist
  • Coordinate, participate in weekly Entry Team meetings and daily shelter update calls
  • Participate in CAN Team meetings
  • Participate in shelter discharge discussions during morning calls
  • Participate in Housing Case Conferences, and DSC meetings as needed
  • Support CAN System Coordinators with Entry and Exit work as needed
  • Position is subject to changes in the scheduled hours (flexibility)
  • Verification Support (Approximately 10% of time)
  • Connect with clients and other social service providers in order to secure verification of chronic homelessness and disability for housing.
  • Work with case managers of the client to assess their housing history
  • Other related duties as assigned

Qualifications

  • High School graduate or GED
  • Excellent organizational and time management skills with a deadline-oriented mindset
  • Ability to analyze and exercise sound judgment
  • Ability to work with electronic databases i.e. Smart sheets, HMIS.
  • Strong written and oral communication skills employing the English language.
  • Able to function as part of a multi-disciplinary team
  • Familiar with mental health, substance abuse and dual diagnosis issues
  • Experience providing person-centered / strength-based services to vulnerable populations.
  • Ability to relate to people of different economic and ethnic backgrounds
  • Able to meet all documentation requirements
  • Knowledgeable about the challenges and barriers that homeless people face
  • Valid drivers’ license with a good driving history
  • Supervised by : Director of Emergency Services and CAN Manager

    Schedule : Monday- Thursday / Tuesday- Friday 9am-5pm

    Salary range : $ $25.50 To $27.50
